The official replacement ballot form. Completed forms can be dropped off at the Clerk and Recorder's Office in the historic Routt County Courthouse. The forms also can be faxed to the office at 970-870-5443. Voters who fax a change-of-address or ballot request form must include the completed, original forms with their mailed ballot.
